- ALL TAX DOCUMENTS RECEIVED IN 2021 INCLUDING THE FOLLOWING:
- Social Security Card/ITIN and cards for all household
- Photo ID-Driver’s License or State ID
- Letter 6419-2021 Total Advance Child Tax Credit Payments
- Letter 6475-2021 Economic Impact Payment
- 1095A-If you received Market Place/Affordable Healthcare
Proof of Income
- Wages-All W-2’s
- Unemployment (Form 1099-G)
- Gambling/Lottery winnings (Form W-2G)
- 1099-Misc.-Miscellaneous Income
- Pension (Form 1099-R)
- Social Security Benefit (Form 1099-SSA)
